Several read/walk failures are silently converted into successful-looking empty data:

  • Hashing errors become a fixed all-zero hash (src/index/pipeline.rs:59-67).
  • Source read errors become an empty string before symbol/import/content indexing (src/index/pipeline.rs:115-118).
  • Filesystem walk errors are skipped (src/discovery/mod.rs:35-39).

A nonexistent project currently produces a successful zero-file index instead of an error. If an existing source file becomes unreadable, an update can replace its indexed symbols and content with empty data. Please propagate errors with the affected path, or explicitly report skipped files; do not silently treat unreadable input as valid empty source.
